The match, staged on Wednesday, February 11 at Kinoru Stadium, ended 2-2 in regulation time, forcing the tie into a penalty shootout.
The two sides battled to a goalless draw in the first half before the contest burst into life after the break.
Luke Namanda opened the scoring for Kuta Collections before the Nairobi-based side doubled their advantage to take firm control of the match.
However, the script flipped in the final 10 minutes.
Melvin Muthomi pulled one back for Al Azizia before Zacharia Tisa converted a late penalty to level matters and send the game into spot kicks.
In the shootout, Kuta missed their first attempt while Al Azizia converted theirs.
But the Division One side faltered under pressure, missing three penalties.
Kuta missed two in total but held their nerve to convert the decisive kick and book their place in the Round of 32.

The side is bankrolled by founder and former FC Talanta midfielder Augustine Kuta.
The cup run marks a historic moment for the Nairobi-based outfit, who are making their debut in the competition.
Kuta, who also marshals the midfield, boasts experience from stints at Nairobi City Stars, FC Talanta, Kibera Black Stars and Equity FC.
His journey through Kenya’s competitive football landscape has shaped the club’s philosophy and ambitions.
Speaking to Mozzart Sport earlier, Kuta said the club’s vision extends beyond immediate results.
“Our main aim at Kuta Collections is to provide talented players with a platform, especially after the transfer window has closed.
We have been around for quite some time, and this season we decided to take things to the next level by competing at a higher standard to give our players an even better platform. That is why we enrolled in the Mozzart Bet Cup,” he explained.
Traditionally, the team has relied on friendly matches within Nairobi to maintain fitness and cohesion.
However, Kuta believes that competing in a structured national tournament offers a greater challenge and exposure for his players.
