With only two points separating the rivals, and both firmly in the Scudetto race, this edition of the derby carries major early-season implications.
Inter Milan sits atop the table after 11 rounds, with AC Milan just behind them in third, and a victory for either side could reshape the complexion of the title battle.
This matchup also arrives with both teams riding very different narratives.
Inter, under Cristian Chivu, are in ruthless form domestically and in Europe, while AC Milan, rejuvenated by the return of Massimiliano Allegri, have built consistency through defensive structure and improved game management.
It sets the stage for another fiercely contested battle between the neighbours who share both a stadium and a long, fiery history.
Inter Milan enter the derby on the back of a strong run, having won seven of their last eight league matches since their early-season wobble.
Their attack has been the most prolific in Serie A, averaging well over two goals per game, and they have scored at least twice in every Champions League fixture so far.
Their 2-0 win over Lazio just before the international break consolidated their position at the summit and reinforced their credentials as early frontrunners.
Their form at San Siro has also been solid, with Inter winning four of their last five home league matches - although both teams scored in three of those encounters, highlighting some occasional defensive lapses.
AC Milan, meanwhile, are enjoying a long unbeaten run across Serie A and the Coppa Italia.
Allegri’s arrival has stabilised a side that was previously struggling for identity, leading to 10 straight matches without defeat.
However, the Rossoneri will feel they dropped points carelessly before the break, surrendering a 2-0 lead at Parma to draw 2-2.
It cost them the temporary top spot and served as a reminder that defensive concentration remains a key concern.
Crucially, Milan are unbeaten in their last five derbies against Inter, winning three of those meetings - including a memorable 3-0 win in the Coppa Italia in April.
Inter’s desire for revenge is palpable, especially after last season’s setbacks against their rivals.
Inter face potential absences in key areas. Denzel Dumfries remains doubtful with an ankle problem, while Henrikh Mkhitaryan and Matteo Darmian are expected to miss out.
In midfield, Chivu will choose between Piotr Zielinski and Petar Sucic to replace Mkhitaryan.
On the flanks, Carlos Augusto may have to play out of position to fill in on the right.
In attack, Inter are spoiled for choice.
Lautaro Martinez, one goal away from reaching double digits against Milan, is expected to lead the line.
He will be partnered by either Marcus Thuram - recently back from injury - or Ange-Yoan Bonny, who has been in scintillating form with four assists and several influential performances.
Milan continues to be without Santiago Gimenez, leaving Rafael Leao and Christian Pulisic to form a fluid but unusual forward pairing.
Christopher Nkunku will likely start from the bench.
The good news for Allegri is the return of Adrien Rabiot, who should partner Luka Modric in midfield.
The biggest selection dilemma is at left-back, where Davide Bartesaghi may keep his spot ahead of Pervis Estupiñán. Mike Maignan, though reliable, has only three clean sheets in 10 Serie A matches, and his form will be essential against the league’s most explosive attack.
AC Milan’s four draws in their last six show that they have struggled to kill off matches. Away from home, Milan have also scored in consecutive fixtures but conceded in both, suggesting defensive inconsistencies that Inter may exploit.
Six of the last seven meetings have seen both teams find the net. Form, statistics, and history all point toward a lively goal-filled contest.
Key Pointers
- Both teams to score (BTTS) has been witnessed in three of Inter’s last five matches
- Inter has seen BTTS occur in three of their last five home league games
- Milan scored and conceded in their last two away fixtures
- Six of the last seven derbies had both teams scoring
- Milan are unbeaten in their last five matches against Inter, winning three
Inter Milan comes into this clash as the stronger attacking side, boasting multiple in-form contributors, while AC Milan continues to depend on efficiency and structure rather than offensive firepower.
Inter’s slight defensive fragility - conceding more than a goal per match on average - could give Milan encouragement, especially with Leao and Pulisic thriving in transitions.
Conversely, Milan’s own defensive inconsistencies, highlighted in their 2-2 draw at Parma, make it unlikely they will keep Inter quiet, especially with Lautaro Martinez in prolific form.
Given the nature of recent derbies, both teams’ attacking strengths, and their defensive vulnerabilities, this matchup once again leans towards goals at both ends. A high-intensity derby with chances for both teams, and goals expected at either end.
Prediction: BTTS (Both Teams To Score - Yes)
